Fecal occult blood refers to blood in the feces that is not visibly apparent. Other types of blood in stool such as melena or hematochezia are apparent in feces.
Fecal occult blood (FOB) refers to the blood that appears in the feces. It is not apparent (unlike other kinds of blood in stool such as hematochezia or melena). A fecal occult blood test (FOBT) checks for hidden blood (occult) in the feces (stool). Latest of these tests look for DNA, hemoglobin, or other blood-related factors involving transferrin, while the conventional stool guaiac tests inquire for heme.

Positive stool (Positive tests) may result due to either lower gastrointestinal bleeding or upper gastrointestinal bleeding and will further investigate for peptic ulcers or a malignancy (colorectal cancer or gastric cancer).
